Please read the following carefully.
I understand that participation in the Taste & See Women's Retreat is voluntary. Although reasonable care will be taken to provide a safe and welcoming environment, attending an event may involve ordinary risks, including slips, falls, allergic reactions, illness, injury, loss or damage to personal belongings, and risks associated with travel to and from the event.
I understand that I am responsible for:
- Taking reasonable care for my own health and safety;
- Informing the event organizers of any allergies, dietary restrictions, medical concerns or accessibility needs that may be relevant;
- Bringing and managing any medications or medical supplies I may require;
- Making food choices appropriate for my own dietary and allergy needs; and
- Securing my personal belongings throughout the retreat.
Release
I voluntarily accept the ordinary risks associated with attending and participating in this event. To the fullest extent permitted by law, I release and hold harmless Open Hands Ministries, Barrhead Alliance Church, the event organizers, speakers, volunteers, ministry representatives and their respective directors, employees and agents from claims arising from my participation in the retreat, except where liability cannot legally be excluded.
Medical Assistance
If I become ill or injured and am unable to provide instructions, I authorize the event organizers to contact emergency services and my emergency contact. I understand that the organizers and volunteers are not medical professionals and that I am responsible for any costs arising from medical care, transportation or treatment.
Personal Belongings
I understand that Open Hands Ministries and Barrhead Alliance Church are not responsible for lost, stolen or damaged personal belongings.
Photography and Video
I understand that photography or video may take place during the retreat for Open Hands Ministries' promotional and ministry purposes. I will notify the organizers if I do not wish to appear in identifiable photographs or video.
Acknowledgment
By signing below—or by selecting the acceptance checkbox during online registration—I confirm that I have read and understood this acknowledgment and waiver; I have had the opportunity to ask questions; I am accepting it voluntarily; and I am at least 18 years old or have provided the required parent or guardian consent.
